Gastroenterology
The Clinic of Gastroenterology and Hepatology is a scientific and clinical division of MONIKI, the main gastroenterological department of the Moscow region. This is also the medical and pedagogical base of FUV MONIKA. The department employs 38 people, including researchers, doctors, nursing and junior medical staff and teaching staff of the Department of Gastroenterology. The diagnostic and treatment work of the clinic is carried out both by full-time doctors and by researchers and teachers of the department. All have the highest or first qualification category; the department employs candidates and doctors of science. The department has 55 beds; rooms for no more than 4 people; There are well-equipped wards for treating patients on a commercial basis. The department deals with the diagnosis and conservative treatment of all diseases of the digestive system.
The most modern laboratory and instrumental methods are used in diagnosis. Treatment of diseases of the digestive system is carried out at a high level in accordance with international and Russian standards and clinical recommendations. The department employs two professors, doctors of science, six candidates of science, and doctors of the highest category. The staff of the department constantly carries out emergency and planned consultations in medical institutions of the Moscow region, and provides methodological assistance to doctors in the region.
Treatment methods
The most complex patients with severe or diagnostically unclear diseases of the esophagus, stomach and duodenum, biliary tract, liver (chronic non-viral hepatitis, drug-induced and toxic liver damage, severe complicated cirrhosis of the liver), pancreas (severe chronic pancreatitis with severe pain syndrome), small intestine (celiac disease, etc.) and large intestine (ulcerative colitis, Crohn's disease, irritable bowel syndrome) from the Moscow region, as well as from Moscow and other regions of Russia.
The most modern laboratory and instrumental methods are used in diagnosis: enzyme immunoassay, polymerase chain reaction, genetic testing, ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), modern endoscopic methods, various immunological and morphological techniques, including immunohistochemistry, morphological diagnosis of helicobacteriosis, puncture biopsy of the liver, fibroelastography of the liver. Intragastric and intraesophageal pH-metry with 24-hour monitoring, sigmoidoscopy, and diagnosis of helicobacteriosis using a non-invasive breath test are performed directly in the department.
Treatment of diseases of the digestive system is carried out at a modern international level based on European, American and Russian standards and clinical recommendations using the latest high medical technologies.
The most severe and diagnostically unclear patients with gastrointestinal pathology from all medical institutions in the Moscow region, often from Moscow and other regions, are hospitalized at the clinic.
A team of highly qualified employees works based on Russian and international standards and clinical recommendations for the treatment of gastroenterological patients, using all the modern diagnostic capabilities of MONIKI. Extremely severe patients with liver cirrhosis, chronic pancreatitis, and intestinal diseases are successfully treated. Some patients with the most complex pathology continue to be observed in the advisory department on an outpatient basis. The clinic constantly provides methodological and advisory assistance to municipal and regional medical institutions; employees respond to emergency calls or consult regional doctors remotely using telemedicine technologies. The staff of the department have extensive experience in participating in international clinical trials of new drugs.
Clinic experience
A regional center for the diagnosis and treatment of inflammatory bowel diseases has been operating at the clinic for many years, concentrating all patients in the region suffering from ulcerative colitis and Crohn’s disease. The most modern high-tech genetically engineered biological drugs are used in the treatment of these serious diseases. Over the past eight years, Russia has accumulated unique and largest experience in the use of these drugs. Algorithms for the treatment of severe hormone-dependent forms of ulcerative colitis and Crohn's disease have been developed. Much attention is paid to the safety of the drugs used, constant monitoring of the effectiveness of treatment is carried out with the selection of individual regimens.
In close cooperation with the surgical clinic, the consultative and outpatient department and the diagnostic departments of the institute, a model for creating a regional pancreatology center based on MONIKI was developed. The main idea of such a center is to form a complete closed complex of examination and treatment of patients, ensuring continuity between the diagnostic process and gastroenterological and surgical care. The center will also deal with methodological issues of providing assistance to these patients in the region, the formation of patient flows and their adequate routing at the municipal and regional levels.
